PERIODONTAL EVALUATION DURING ORTHODONTIC TREATMENT WITH ALIGNERS AND FIXED APPLIANCES

Abstract

The growing demand for orthodontic treatment has led to the development of more comfortable and aesthetic alternatives, such as orthodontic aligners. In addition to aesthetics, their removal makes them easier to clean than fixed appliances. The aim of this study is to evaluate, through a narrative review of the literature, the periodontal profile of patients who use aligners and fixed appliances. To conclude, a bibliographic search was carried out using the electronic databases LILACS, PubMed, Scopus, SciELO and Web of Science. The search was carried out until March 2025, with a time frame of 5 years, from 2020 to 2025, with no language limitation. Clinical studies were included which assessed the following parameters (PI, GI or SS and PSC) in healthy patients during orthodontic treatment with aligners or conventional fixed appliances, with at least two evaluation times before or during the use of the appliances. A total of 364 studies were screened, and 13 articles were included. The results indicate that aligners favor better oral hygiene control, with less impact on periodontal health. However, studies that implemented oral hygiene reinforcements during treatment with fixed appliances showed an improvement in periodontal indices. Thus, maintaining periodontal health depends not only on the type of appliance used, but also on effective preventive strategies. However, the literature is not unanimous. Therefore, future long-term clinical studies are needed to establish clearer conclusions about the impact of different types of orthodontic appliances on periodontal health.
